Analysis of saccular aneurysms in the Barrow Ruptured Aneurysm Trial

Published ahead of print last week online in the Journal of Neurosurgery the Barrow Ruptured Aneurysm Trial (BRAT) published their subgroup analysis of only the saccular aneurysms included in the study. The BRAT6 year results have previously been published however this subgroup analysis included review of only those patients enrolled in BRAT who had subarachnoid hemorrhage related to saccular aneurysms.

Of the initial 471 patients enrolled in the study, 77% (362/471) had saccular aneurysms. In the saccular aneurysm subgroup, there was no significant difference in outcomes from discharge through the 6 year follow up. In addition, retreatment was more frequent in the coiling treatment group, with 21 retreatment’s performed (18%) compared to the clipping arm which had 1 retreatment (0.4%). Complete obliteration rates also favored clipping in this analysis with 96% of clipped aneurysms having complete occlusion, compared to 48% of the coiled aneurysms.
